The Benefits unit assists faculty, staff, and graduate students in
understanding their benefit programs. We work closely with the Department
of Civil Service, Division of
Employee Benefits, SUNY
System Administration, and the various carriers to ensure that the
programs work smoothly and employees receive their maximum entitlement.

for prior authorizations. A
dedicated phone line has been established to better assistance you. The
Empire Plan notification number is
1-888-333-9067. It may be advisable to have your doctor call
on your behalf to provide codes and diagnosis. For additional
