Q. Where can I get information on Professional Service employment procedures?
A. Complete information on Professional Service employment procedures may be obtained by going to Professional Employment Procedures.

Q. What is an I-9 and why do I have to show originals as supporting documentation?
A. The I-9 is a federally required document which verifies that an individual is eligible for employment in this country. According to the rules governing the completion of the I-9, only original documents can be used.

Q. What documentation is needed to complete the I-9?
A. Depending on your status, you can find the required documentation listed on the back of the I-9 form: one document from list "A" OR one each from lists "B" and "C".

Q. Where do I go to get an I-9 completed?
A. It is highly recommended that international employees come to UAB-300 to ensure that the I-9 is completed properly.

All other employees should check with the departmental secretary for a person authorized to complete the I-9 form.

Q. How often do I have to complete an I-9 form?
A. U.S. citizens: I-9's are valid continuously unless a break of more than year of employment occurs.

International employees on F-1 (student) visas must renew their I-9 each year. Those on H-1B (speciality occupation) or J-1 (exchange visitor) visas must have their I-9 updated each time their visa is renewed.

Q. Why must I complete a Change of Status Request (HRM-3) for extra service, since this form does not reference a box for extra service? In what box should I place the information pertaining to the extra service?
A. The HRM-3 form is used to report all changes in status at the University. An extra service appointment is considered an additional obligation. Therefore, it should be noted under Change(s) in Terms of Appointment on the HRM-3 form. The salary for the extra service should be placed in the box captioned New Salary and the words Extra Service should be included. The dates for the extra service should be placed in the Effective Date and Ending Date boxes.
