The preferred method for submitting an Announcement of Professional Vacancy (Form UP-5) electronically is to download and complete the Microsoft Word™ UP-5 template from the OHRM website. After downloading and completing the template, you should save the form on your local computer and then send it as an attachment to email to the Office of Diversity and Affirmative Action (ODAA) for review.  In the event that you do not have access to the Microsoft Word™ program to complete the template, you can provide the same information to ODAA by completing the UP-5 online form.
